Vybrané kapitoly z gynekológie a pôrodníctva pre študentov fyzioterapie

Otázky človeka, jeho bytia, násilie znepokojujú ľudí od najdávnejších čias ľudskej spoločnosti a sú nemalými problémami aj v prítomnosti. V tejto pôvodnej práci, ktorej predchádzali viaceré štúdie autora (najpodrobnejšie Kafka, 2002), sa zameriavam na opísanie násilia vo všetkých základných formách, čo je jednou z jej osobitostí. Podávam operačný výmer násilia:

Násilie je vedomá ľudská činnosť využívajúca ako prostriedok násilného činu fyzickú alebo psychickú silu na ovplyvnenie druhého človeka (pri pôsobení na seba ide o zvláštny prípad násilia), ktorý potom koná pod vplyvom tohto pôsobenia nedobrovoľne, z donútenia.

Nazdávam sa, že násilie charakterizuje určitá situácia, v ktorej sa stretávajú najmenej dvaja ľudia: aktér násilia a obeť násilia, alebo sa stretajú dvaja aktéri a jeden z nich sa zvyčajne stane obeťou. To je však osobitný prípad, ktorým sa teraz nebudem zaoberať. Ak je násilie zamerané proti vlastnej osobe, je aktér násilia súčasne aj obeťou. Aktér, ktorého osobnosť a motívy bývajú rôzne, začína násilný čin využívajúc pritom viaceré prostriedky a pôsobí nimi na druhého človeka, ktorého núti vykonávať svoju vôľu, čím sa tento stáva obeťou. Celý akt má určité následky. Napadnutý sa môže brániť tak, že násilie zvládne alebo to môže skončiť tragicky, keď násiliu podľahne. Táto relatívne jednoduchá reakcia sa stáva zložitejšou pri skupinovej forme násilia a celkom neprehľadnou pri hromadnom násilí.

Opisujem tieto základné formy násilia:

• - násilie proti vlastnej osobe vrátane samovraždy,
• - násilie individuálne, spôsobené jedným aktérom násilia,
• - násilie skupinové, charakterizované malými skupinami,
• - násilie hromadné alebo kolektívne medzi veľkými skupinami, vrátane vojen a revolúcií,
• - násilie proti neživej a živej prírode.

Práca končí krátkou analýzou liečby násilného správania a dôsledkov násilia s možnosťami jej prevencie. Opieram sa aj o vlastné skúsenosti s prejavmi násilia v psychiatrických zariadeniach. Nazdávam sa, že je to práca veľmi aktuálna, informatívna, ale aj pôvodná, lebo podáva v koncentrovanej podobe celý rozsiahly fenomén násilia s jeho základnými charakteristikami. Podobnú prácu v našej literatúre nemáme.

The scientific monograph is the output of the Care4Youth project, which aimed to map the current system of care for adolescents with emotional and behavioral problems (EBP) in Slovakia. It captures what the care system is and what its possible shortcomings are from the perspective of all those affected (adolescents with problems, their parents, care providers and other institutions involved).

It also looks at the trajectory of adolescents with EBP in the care system, i.e. when and how adolescents with problems enter and leave the care system, or pass from one type of care to another and what impact different trajectories have on them.

Last but not least, the aim of the monograph was to provide expert information on what are the risk and protective factors that will affect their success in the care system or, conversely, that threaten them. All this in order to support the proposal for improvements in the care system that would benefit all stakeholders.

The scientific program of the spring school will comprise 3 plenary lectures by leading scientific experts and 1 workshop. Doctoral students will present their research papers in two sections: 22 doctoral students of Faculty of Public Administration, Faculty of Law and Faculty of Arts, and 26 doctoral students of Faculty of Medicine and Faculty of Science. The scientific programme of the spring school will also include a panel discussion with the management of Pavol Jozef Šafárik University in Košice.

The premise “today’s research shapes the healthcare of tomorrow” expresses the importance of medical research and the reason for its pursuit. Scientific research and advances in biomedical sciences, as well as their application in medical practice and in the process of delivering healthcare, nevertheless bring new problems in the area of ethics.

Biomedical research itself requires the precise definition of hypotheses, which are tested both under laboratory conditions and on experimental animals. Although clinical studies on human subjects are expected to benefit individual participants and to advance fundamental biomedical knowledge, they also entail certain risks depending on the nature of the research. These risks must be minimized, and the experiment should be conducted freely while also taking place within specific measures to protect all involved-not only the test subjects, but also the researchers themselves.

The presented publication provides an overview of the general conditions and ethical principles of research involving laboratory animals and human subjects, the issue of professional and ethical assessment of projects, as well as fraud and the publication of results. At present, we are witnessing tremendous progress in molecular biological sciences, which brings ethical dilemmas also in the field of genetics. For this reason, one of the chapters is devoted to the latest findings in this area.

Current classification of demyelinating diseases of the central nervous system according to van der Knaap and Valko from 2005 includes 97 congenital and acquired diseases that cause demyelination of the white matter of the brain and spinal cord. Most of these disorders lacks any definitive diagnostic test, so the accurate diagnosis depends on upon clinical symptoms, course of the disease, neurological findings, and the results of neuroimaging and laboratory examinations.

The aim of this textbook is to provide an accurate overview of acquired demyelinating diseases of the CNS, their etiopathogenesis, clinical picture and diagnostic options.

Both clinical and radiation oncology are the most developing parts of medicine. Regardless of the physician´s specialization, all of them will have to face subjects with cancer in their daily clinical practice. It includes not only the treatment itself, but also obtaining the diagnosis, but also differential diagnosis and treatment of different adverse events caused by oncological treatment. This publication provides basic data about tumor characteristics necessary for oncologists to determine the final stage and choose the best treatment strategy. In addition, explains the basic oncological terms, forms of therapy, and provides information about treatment timing, combination strategies and expected outcomes. It explains main endpoints of clinical trials performed in oncology to understand them.  In the second part this publication describes mechanism of action of all treatment modalities used in clinical oncology, provides information about potential side effects and basic knowledge how to deal with them. The last chapter is focused on palliative care, how to relieve symptoms of dying patient and how to deal with cancer-related pain.

Predkladaná monografia sa komplexne venuje problematike urolitiázy. Má 19 kapitol, ktoré zahrňujú celú šírku problematiky urolitiázy a to epidemiológiu, etiológiu, typy konkrementov, osobitné situácie pri urolitiáze, diagnostiku urolitiázy, zobrazovacie vyšetrovacie metódy pri urolitiáze, konzervatívnu liečbu urolitiázy, chirurgické a endoskopické metódy liečby urolitiázy, metafylaxiu konkrementov a konkrementy v močovom mechúri. V monografii sa zdôrazňuje význam individuálneho rozhodovania o optimálnom spôsobe liečby. Do úvahy sa berie veľkosť konkrementov, ich počet, lokalizácia, anatomické predpoklady a röntgenologické charakteristiky. Pri plánovaní aktívnej liečby sa zdôrazňuje význam resp. potreba zobrazovacími metódami vyšetriť aj anatómiu vývodných močových ciest. Nezastupiteľné miesto, najmä v prípadoch viacpočetnej a opakujúcej sa urolitiázy a geneticky podmienenej urolitiázy, má kompletné metabolické vyšetrenie. Dokumentuje všetky moderné spôsoby diagnostiky a liečby urolitázy. Metodicky opisuje liečbu urolitiázy – akútny management pri renálnej kolike, pri urosepse pri konkrementom zablokovanej obličke a v tehotenstve. Prevažná väčšina pacientov s urolitiázou je liečená farmakologicky a semininvazívnymi technikami. Vychádza sa z vlastných skúseností, komentuje využitie extrakorporálnej litotripsie, endoskopických metód, stentov. Monografia venuje pozornosť aj auxiliárnym metódam, ktoré sa používajú v liečbe urolitiázy, liečbe sprievodnej infekcie, poranení obličiek a močových ciest, poranení okolitých orgánov, rôznych spôsobov derivácie moču, krvácania. V závere sú spomenuté kalcifikácie pri obličkových tumoroch.

„Patológia 1. časť (zimný semester) - Praktické cvičenia pre študentov všeobecného lekárstva“ je prvou časťou vysokoškolskej učebnice predstavujúcou prvú polovicu výučby (zimný semester). V tejto časti je popísaných 70 diagnóz, kde každej je venovaná jedna strana. V popise je uvedené poradové číslo zhodujúce sa s číslom preparátu a názov danej diagnózy. Študent sa pri každom preparáte oboznámi so základnými informáciami o danej chorobe, jej makroskopickým a mikroskopickým obrazom. Navyše je vytvorené miesto, v ktorom si študent môže zaznačiť (nakresliť) charakteristický mikroskopický obraz danej choroby. Preparáty v tejto učebnici sú zamerané na všeobecnú patológiu – regresívne a progresívne zmeny, pigmentáciu, hojenie, zápaly a nádory.

Tým, že učebnica umožňuje preparáty zakresľovať, vytvára študentom priestor pre trvalejšie zapamätanie si danej problematiky. Je vynikajúcim zdrojom pre prípravu študentov na kolokvium a ústnu skúšku.

Although chronic non-communicable diseases currently dominate overall morbidity and mortality in developed countries, epidemics of infectious diseases, especially those caused by new or emerging pathogens, occur more frequently and spread faster and further in new territories. The starting factors of this threat are mainly ecological and climatic changes and changes in human demography and behaviour. The growing resistance to ATB is also very serious, as a result of which there is a significant increase in the incidence of infectious diseases and their spread to new geographical locations. Current problems of epidemiology and microbiology in research and clinical practice are mainly focused on rapid detection and subsequent response to diagnosed diseases. The correct diagnosis of an infectious disease is the result of a clinical picture, knowledge of the current epidemiological situation and laboratory confirmation of the etiological agent. The identification of microorganisms allows us to establish the correct definitive diagnosis and subsequent targeted therapy, but also to start immediately with appropriate anti-epidemic measures to control the spread of the infectious agent.
