Predkladaná monografia sa komplexne venuje problematike urolitiázy. Má 19 kapitol, ktoré zahrňujú celú šírku problematiky urolitiázy a to epidemiológiu, etiológiu, typy konkrementov, osobitné situácie pri urolitiáze, diagnostiku urolitiázy, zobrazovacie vyšetrovacie metódy pri urolitiáze, konzervatívnu liečbu urolitiázy, chirurgické a endoskopické metódy liečby urolitiázy, metafylaxiu konkrementov a konkrementy v močovom mechúri. V monografii sa zdôrazňuje význam individuálneho rozhodovania o optimálnom spôsobe liečby. Do úvahy sa berie veľkosť konkrementov, ich počet, lokalizácia, anatomické predpoklady a röntgenologické charakteristiky. Pri plánovaní aktívnej liečby sa zdôrazňuje význam resp. potreba zobrazovacími metódami vyšetriť aj anatómiu vývodných močových ciest. Nezastupiteľné miesto, najmä v prípadoch viacpočetnej a opakujúcej sa urolitiázy a geneticky podmienenej urolitiázy, má kompletné metabolické vyšetrenie. Dokumentuje všetky moderné spôsoby diagnostiky a liečby urolitázy. Metodicky opisuje liečbu urolitiázy – akútny management pri renálnej kolike, pri urosepse pri konkrementom zablokovanej obličke a v tehotenstve. Prevažná väčšina pacientov s urolitiázou je liečená farmakologicky a semininvazívnymi technikami. Vychádza sa z vlastných skúseností, komentuje využitie extrakorporálnej litotripsie, endoskopických metód, stentov. Monografia venuje pozornosť aj auxiliárnym metódam, ktoré sa používajú v liečbe urolitiázy, liečbe sprievodnej infekcie, poranení obličiek a močových ciest, poranení okolitých orgánov, rôznych spôsobov derivácie moču, krvácania. V závere sú spomenuté kalcifikácie pri obličkových tumoroch.

V predloženej učebnici si autori predsavzali zhrnúť informácie, ktoré pre úspešné absolvovanie predmetu Anestéziológia a intenzívna medicína považujú za esenciálne. Množstvo textu je stranovo limitované, preto si nemožno nárokovať na kompletnosť zhrnutej problematiky. Predpokladá sa, že študent na preukázanie vedomostí v študijnom predmete Anestéziológia a intenzívna medicína využije aj poznatky a skúsenosti nadobudnuté počas prednášok, praktických cvičení a samostatného štúdia iných dostupných zdrojov podľa zverejneného obsahu okruhov tém vyžadovaných na skúšku.

Určite si však v nej nájdu zdroje mnohých informácií aj sestry pripravujúce sa na skúšku v postgraduálnom špecializačnom štúdiu v odbore Anestéziológia a intenzívna starostlivosť.

Clinical Biochemistry - selected chapters - MEFANET >>> 

After 7 years, we finally reworked the textbook on the subject of clinical biochemistry at UPJŠ FM in Košice. We have innovated information in accordance with recommendations of professional medical societies and added new chapters. As the scope of the subject is limited, the textbook does not cover all issues of biochemical diagnostics. It focuses on disorders of water-ion and acid-base balance disorders, and basic laboratory diagnostics of diseases of selected organs.

The textbook of clinical biochemistry has the ambition to become a practical aid for senior medical students and beginning doctors in the study of laboratory medicine. It should make it easier for them to orientate themselves in routinely used clinical-biochemical examinations and could form a springboard for a more detailed studying of laboratory methods and their use in diagnostic and monitoring algorithms.
Like any medical discipline, clinical biochemistry is evolving rapidly and gradually integrating with other areas of laboratory medicine. New diagnostic methods penetrate deeper into the essence of the biochemical mechanisms of the human body, and "omic" methods are gaining ground. It is almost sure that in a few years, laboratory examination looks quite different from today. If this publication quite hypothetically reaches its next edition, many chapters will need to updated, and new chapters written following the up-to-date status.

Nature, as well as the human body, contains an abundant quantity of organic molecules. This textbook describes selected oxygen, sulfur, nitrogen derivatives of hydrocarbons, heterocyclic compounds of their reactions, which are important in the human body or are found in pharmaceutical active substances, poisons, foodstuffs and heat-treated food. The last part of the textbook describes the isomerism of organic compounds, which enables dynamic changes in the structure of molecules that have different properties. Nature is filled with different plants and animals and is the first laboratory to produce bioactive organic natural compounds. These compounds are compatible with human cells and are applicable as therapeutic medicine. There is an effort to reproduce natural molecules in chemical laboratories, but they cannot be as perfect as the natural ones. Synthetic molecules can serve as a model for multiple studies, but there still does not exist a magic panacea pill or a perfect copy of nature, for the treatment of various diseases. Knowledge about the regenerative and toxic effect of medicinal substances in dependence on their structure (type and number of substituents) is very important for the understanding of their role in modulation of health and disease in dependence on their concentration. Compounds can be used daily only in recommended doses (higher doses can be toxic), but still will not replace a healthy diet. People need a varied diet to be healthy, in which are all necessary nutrients, what Hippocrates demonstrated long ago “Let food be the medicine, and let medicine be the food.” This book is written for students who select bioorganic chemistry subject during the study of general medicine or dentistry as well as for motivated students who would like to practice their knowledge and wish to improve in this subject and for students who would like to learn simplified basic organic reactions with their future application in biochemistry. Learn the simple models mainly through creative simple instruction by using different colours, by repetition, practice by drawing of formulas by hand (at least three times) and memorization of common names of compounds and their reactions which have application in biochemistry. Thus, establishing simple educational methods how to distinguish organic molecules according to the colours of a functional group can promote universal learning for all students who thought that chemistry is a difficult subject, but hopefully they change their opinion after study of this book.

Ethics, as the science of the proper realization of human potential in various areas of life, also extends into the environment. For understanding the ethical aspects in this field, a thorough knowledge of the individual components of the environment, as well as their impact not only on humans but on all living nature, is very important. Therefore, questions concerning the influence of the environment are increasingly at the forefront of interest from an ethical perspective, also in connection with existing legal norms.

The modern process of civilization is characterized by rapid development of science and technology. However, this development, while raising the standard of living on one hand, also brings negative side effects on the other. The result is the disruption of ecological, biological, and natural life conditions, especially in the areas of industrial centers. The development of individual branches of industry leads to the production of liquid, gaseous, and solid wastes, the emergence of harmful risk factors threatening various components of living nature, including humans. There may be increased bioaccumulation of hazardous chemical elements in organisms, which, through their effects, represent a potential risk also in terms of carcinogenesis and mutagenesis.

For this reason, an important role of society is to create conditions to prevent the release of emissions and to reduce their concentration to the lowest possible, socially and biologically acceptable level. The assessment of the biological effects of harmful substances is based on experimental and epidemiological studies. Knowledge of concentration and duration of exposure is crucial for applying dose-effect relationships and for determining the highest permissible concentrations of pollutants. The health of the population, as a universal value and a primary human right, represents a fundamental economic resource not only for the individual, but also for society as a whole.

Laparoskopická chirurgia dnes predstavuje štandardnú a v mnohých ohľadoch preferovanú modalitu chirurgickej liečby. Dynamika jej celosvetového rozšírenia, umožnená pokrokom v biomedicínskych technológiách a rozvojom globálnych informačných a komunikačných kanálov, nemá v histórii chirurgie obdobu. V súčasnosti už prakticky neexistuje žiadna brušná operácia, ktorá by sa nevykonávala laparoskopickým spôsobom, pričom v mnohých prípadoch je už laparoskopický postup metódou voľby.

Napriek, a možno práve vďaka bezprecedentne rýchlej penetrácii laparoskopických operácií do každodennej chirurgickej praxe naďalej v mnohých ohľadoch absentujú všeobecne akceptované technické štandardy. Vysoká variabilita procesných postupov v rámci jednotlivých laparoskopických operácií vychádza skôr zo zvyklostí daného pracoviska ako z medicíny založenej na dôkazoch. Táto situácia je ďalej modifi kovaná rozvojom nových a staronových koncepcií charakteru minilaparoskopie, SILS či NOTES a ich hybridných kombinácií s klasickou laparoskopiou.

Cieľom monografi e je pritiahnuť pozornosť odbornej verejnosti späť k základným princípom laparoskopickej chirurgie, ktoré by mali byť základňou pre efektívne a bezpečné vykonávanie laparoskopických operácií, ako aj bázou pre kritický postoj k módnym inovatívnym trendom. Obsahovo sa sústredí skôr na všeobecne menej známe a v našich podmienkach zriedkavo diskutované témy, ktorých význam sa hlbšie analyzoval len v nedávnej minulosti. Našou ambíciou bolo vytvoriť vyvážený materiál, ktorý by bol zaujímavým primárnym informačným zdrojom nielen pre lekárov špecialistov, ale aj pre poslucháčov lekárskych fakúlt a kolegov v špecializačnej príprave v chirurgických odboroch.

Výučbový materiál môže poslúžiť nielen ako príprava k praktickým cvičeniam so simulátorom Cardiology Patient Simulator „K“ ver 2 (Kyoto Kagaku), ale aj ako užitočná pomôcka pre študentov počas praktických cvičení pri lôžku pacienta. Študenti v publikácii nájdu okrem detailného opisu auskultačných nálezov na jednotlivých chlopniach aj podrobne vysvetlenú ich klinickú manifestáciu u pacienta pri fyzikálnom vyšetrení. The textbook provides a clear view of the importance of respiratory care, with an emphasis on prevention and treatment procedures. The introductory chapters present the anatomical and physiological principles of respiratory function necessary for understanding further contexts of the issue. Methods for facilitating expectoration of mucus and foreign bodies are explained. Great emphasis is placed on correct procedures for humidifying and warming the inhaled mixture, including the basics of aerosol therapy. This is an issue that is particularly important in invasive, but also non-invasive methods of ventilation therapy. The work also contains more detailed physical justifications for the explained principles. From a practical perspective, the chapters that draw attention to frequent errors in the use of humidification and aerosol technologies are also important. The texts are supplemented with many illustrative original images and explanations.

The textbook is intended primarily for doctors, nurses and physiotherapists working in the fields of anaesthesiology and intensive care, emergency medicine, in all workplaces that are dedicated to the treatment and care of patients with respiratory problems, such as pneumology, internal medicine, surgical units treating patients in the postoperative period. University scripts “Visceral pathology of vascular origin“analyze one of the most challenging vascular regions, the aorta and the visceral aortic branches, from the surgical perspective. The diagnostic and the treatment aneurysms of the abdominal aorta are dedicated to the first chapter. Early diagnostic and operative treatment is crucial in the treatment of ruptured aneurysms of the abdominal aorta. Shortening of the interval between the patient’s arrival with ruptured aneurysm of the abdominal aorta into the hospital and the beginning of the operation decreased the perioperative mortality. This shortening, together with the use of the cell savers, led to the dropping of perioperative mortality from 50% to 20%. The diagnostics and surgical treatment of the acute and chronic occlusion of the aorta and iliac arteries are analyzed in the second chapter. In this chapter, the focus is given to the aortobifemoral bypass as “the gold standard“ of the surgical therapy of aortoiliac atherosclerosis. The management of acute mesenteric ischemia is the content of the third chapter. Acute mesenteric ischemia is a severe condition with high mortality. The problem of acute mesenterial ischemia is a delayed diagnostic that is the reason for poor therapeutical outcomes of the disease. The possible imagine studies and operations used in treating acute mesenteric ischemia are brought in this chapter. Chronic mesenteric ischemia leads to cachexia. The diagnosis of chronic mesenterial ischemia is one of the most demanding, and surgical therapy used in the treatment of chronic mesenterial ischemia also belongs to the most challenging operations. Left-sided colon ischemia belongs to the severe complications of the aortoiliac reconstructions. Left-sided colon ischemia has a high mortality. In the prevention of the left-sided colon, ischemia is preserving at least one internal iliac artery. The complex diagnostics treatment of intraabdominal compressive syndromes as Dunbar syndróme, „Nutcracker“ syndróme a Wilkie syndróme, analyzed in the last chapter.

Particularly accentuated are issues related to specific effective communication skills, methods, and techniques serviceable during interactions with a patient; application of empathy and proactive approach are highlighted.
